  • We're here to ensure that you receive a positive post-operative care experience. Our program has one goal: To help you get well soon by regaining your independence and maintaining your quality of life.

  • We follow up with providers throughout a patient's stay, and we act as a liaison between the provider, the patient and the patient's family to help make the patient's transition from health care facility to Touchpoints to home as seamless and comfortable as possible.

  • As members of the Touchpoints team, we are advocates, caregivers and friends to our patients, and this relationship extends beyond your stay in our facility.
