Noble hospital will destroy patients' records from 1993

SPRINGFIELD, MA (WSHM) -

On March 15, 2013, Noble Hospital will destroy all records of patients who last visited before Dec. 31, 1993.

Commonwealth of Massachusetts' rules allow medical records to be kept for up to 20 years.

All records will be retained entirely for those who have been patients between Jan. 1, 1994 and Wednesday, Jan. 30.

For any questions, or to request a copy of your records, contact Don Wright at 413-572-5156.

All mail-in requests should be send to Noble Hospital PO Box 1634. Westfield, MA 01086-1634. Also include, the date of your request, name at the time of your visit, current name (if different), your date of birth and your signature.

For additional copies, there will be a charge of 60 cents per page.
