Fire officials said the water main break was reported at the Avalon rehabilitation center, next to the Stoneridge facility, on Jerry Browne Road.
Fire crews said 36 patients were evacuated, and taken to the auditorium at Stoneridge.
Major water damage has been reported in three wings of the facility.
No injuries were reported.
Ambulances from southeast Connecticut and Rhode Island responded to the scene to assist, and officials said the health department was also expected to respond.
